Explanation: Even though this patient is septic, whenever possible, a confirmed or suspected source of sepsis should be documented. Why? "Sepsis" and "empiric" indications do not help us communicate likely causative organisms and therefore appropriate antibiotic selection, dosing or eventual length of therapy. In this patient there are clues that our source is likely intra-abdominal. Terms like "sepsis secondary to an intra-abdominal source" can convey both the source and severity of infection. Sometimes the source is unclear and a more general indication must be used initially. The true source should still be documented whenever it is defined to aid in optimizing definitive therapy.
